Takes about ten minutes on your phone. It saves time when you arrive and gives us a head start on your history. Start it on the Patient Tools page.
Arrive a few minutes early
We're downtown at 5014 48 St. Free parking out back. Bring your insurance card and a list of any medications.
Full exam + cleaning
Plan for 60-90 minutes. We'll do a full exam including an oral cancer screening, digital X-rays where appropriate, and a cleaning if you're due. No pressure, no upselling — we'll talk through what we find and what we'd do next.
Written treatment plan
You'll leave with a plain-language plan and rough cost estimate. If anything needs insurance predetermination, we'll handle the paperwork and let you know before you commit to anything.
